Neil Asher Continuing Education for Manual Therapists PRO — "Neil Asher Technique" is branded approach to trigger point therapy, and the website is mostly built around a directory of NAT certified therapists.|Evidence for the use of medications in trigger point management is lacking.7 Other less invasive treatments include massage, osteopathic manual medicine, physical therapy (PT), and the spray and stretch technique. Nonpharmacologic treatment modalities for trigger points have been studied, but no standardized treatment protocol has been established.2 Trigger point treatments include oral nonsteroidal anti-inflammatory drugs, acetaminophen, and muscle relaxants. During the palpation portion of the examination, trigger points are elicited when the patient experiences pain while the physician palpates a band-like nodule within the muscle.2 In patients with musculoskeletal pain, especially in the neck or back, identifying trigger points should be part of a detailed history and neuromuscular examination. Typical locations for trapezius trigger points are pictured in Figure 1.2,6 Gluteus maximus, gluteus medius, and quadratus lumborum muscles also commonly harbor trigger points.|The multi-dose metered pump provides 12.5 mg of testosterone per actuation. Vogelxo® 1% gel is available in a multi-dose metered pump, unit-dose tubes and packets. Monitoring of serum testosterone should be performed 14 days after starting Testim®. The subjects applied Fortesta® 2% gel initially at 40 mg/day to their front and inner thighs for 90 days.} These large fluctuations in serum testosterone over a 2-week period illustrate the less than ideal kinetics of TC IM injections. A PK study evaluated serum levels of testosterone periodically for 14 days after administration of TC 200 mg IM in 11 hypogonadal men (42). Normal physiological range of serum testosterone was defined as 300–1,050 ng/dL, and doses were adjusted on days 45 and 90. A multicenter, open label study in men with documented androgen deficiency was conducted to evaluate the effects of Axiron® at a dose of 60 mg/day (38). Only six of 205 subjects in the gel groups discontinued therapy; two were related to drug specific adverse effects (hypertension and mood swings). On day 60, patients subtherapeutic in the 50 mg gel group were increased to 100 mg/day, while supratherapeutic subjects in the 100 mg group were decreased to 50 mg/day. Adverse effects were reported from a controlled clinical study where 304 patients were treated with Testim® 50 mg, 100 mg or placebo up to 90 days (33). When the tip of the needle touches the trigger point, you may feel a brief increase in your pain. There’s nothing you need to do to prepare for a trigger point injection. A trigger point affecting your piriformis muscle (a muscle in your buttocks) can cause piriformis syndrome — when the piriformis muscle presses on your sciatic nerve. For example, trigger points affecting your trapezius muscle may cause tension headaches. The trigger point injection can allow physical therapy to be more effective. Complications of trigger point injection and dry needling are rare; however, serious injuries have occurred, including pneumothorax and spinal cord injury. In a recent RCT, 168 patients with chronic tension-type headache were randomized to receive dry needling or sham dry needling delivered in three sessions over two weeks.27 The sham dry needling protocol involved dry needling into adipose tissue in an area where an active trigger point was absent. This is an example of a strong placebo-type effect that is commonly observed in trigger point injection trials. Trigger point injection has also been employed in the treatment of temporomandibular joint disorders, pelvic floor pain, and headaches. Following a post-isometric relaxation phase, the muscle is then stretched to a new point of restriction; this is repeated for three to five cycles.18 Counter strain is a manual technique involving the manipulation of joints and muscles away from a restrictive barrier toward a position of ease until trigger point pain is reduced. A similar RCT containing 62 patients with tension-type headache compared massage with sham massage over 12 sessions.10 A statistically significant improvement in pain-pressure threshold was observed over the trapezius and suboccipital muscles in the massage-treated group, representing an increased ability to tolerate pain. "Eating a diet with high saturated fats and processed sugars can lead to caloric excess and weight gain that can lower testosterone." Autoimmune diseases, such as rheumatoid arthritis and lupus, are also linked with testosterone deficiency. This might include Kallmann’s syndrome, which is a brain development abnormality, or a pituitary disorder that might interfere with normal testosterone production, according to the Mayo Clinic. For beginners with average body pain — a typical case of unexplained nagging hip pain or low back pain or neck pain — the advice given here may well seem almost miraculously useful. There are no "trigger point whisperers." Trigger points are not little switches that can be flicked off ("released") by anyone who has sufficiently advanced technique — they are a mysterious, cantankerous, complex phenomenon. They are often barking up the wrong tree, treating so-called trigger points when there’s actually another problem. If so, it’s no wonder they hurt … and hurt weird. Time-course differences are likely attributed to varying testosterone preparation pharmacokinetics (PKs). Positive effects of TRT show considerable variation in terms of length of time to detection. In practice, initiation of TRT is usually driven by patient symptoms in addition to low serum level. While there is no consensus as to what serum testosterone level to initiate TRT (3).
